Mobilizing for Action Through Planning and Partnerships (MAPP)

MAPP is a process by which the Union County Office of Health Management will facilitate the development of a Community Health Improvement Plan. This plan will prioritize health indicators and issues in our community and will provide a roadmap by which future program development will be conducted. Community Health Improvement Plan (CHIP)

The Community Health Improvement Plan (CHIP) reflects the top four public health priorities within Union County. The health priorities were derived from the strategic health planning process known as Mobilizing for Action through Planning and Partnerships (MAPP). A committee has been developed to work on the priority issues and has developed an action plan in order to resolve or make a positive impact on these issues. The CHIP is designed to be utilized as a resource document for agencies looking for comprehensive information on the health needs of Union County residents.

The Office of Health Management collaborated with numerous stakeholders, community residents, and agencies within the County. The cooperation with these community partners made this effort successful.
