To check the status of your application, you will need your:

• Last Name
  • Include suffixes (Jones III, Patton Jr, ect.)
  • Do not include punctuation (O Brien, Carter Jr, ect.)
  • Do include hyphens as appropriate (Jackson-Smith)
• Date of Birth (MM/DD/YYYY)
• Last Four Digits of Social Securty Number
