As of February 1, 2008, the US Department of State introduced a new travel document for US citizens called the US Passport Card. This card is valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda, and the Caribbean only. Citizens traveling by sea are urged to confirm with their cruise line to determine if any other ID is required for travel. If traveling by air, citizens must apply for a Passport Book. Citizens may apply for both travel documents, however the Passport Card will not be available until Spring 2008.
Passport Book
Union County residents may obtain a United States Passport for foreign travel. The following items are required to obtain a Passport:
An original birth certificate issued by Vital Statistics/City Hall
with a raised seal and date of issue;
A naturalization or citizenship certificate, if applicable;
An old U.S. Passport, if any, is preferred over any of the above;
Two recent professional 2" x 2" photographs, front facial pose
(available at office for $10.00);
Total fee is $100.00, of which $75.00 must be by check or money order payable to US Department of State, and a $25.00 execution fee payable by cash, check, or money order to the County Clerk;
For parents, a new Federal law applies for children under 16. Please call for requirements.
For children under the age of 16 the fee is $60.00 plus $25.00 execution fee;
A valid driver's license or three types of identification and a witness.
Passport Card
Union County residents may obtain a United States Passport Card valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean only. This card is not for travel by air. All passport requirments are the same for passport cards. The fees are as follows:
Total fee is $45.00, of which $20.00 must be by check or money order payable to US Department of State, and a $25.00 execution fee payable by cash, check, or money order to the County Clerk;
For children under the age of 16, total fee is $35.00, of which $10.00 must be by check or money order payable to US Department of State plus $25.00 execution fee payable by cash, check, or money order to the County Clerk;
This form must be presented and signed in person at an authorized passport acceptance facility. If you have never had a U.S. passport or if your most recent passport was lost or stolen you must use this form. If you are applying because your passport was lost or stolen you will also need to submit Form DS-64, located below.
This form should be submitted by mail if you are renewing a previously issued U.S. passport. There are four criteria for using this form. Please use the following checklist to see if you are eligible to apply with a DS-82:
I can submit my most recent passport
I was at least 16 years old when my most recent passport was issued
I was issued my most recent passport less than 15 years ago
I use the same name as on my recent passport, OR, I have had my name changed by marriage or court order and can submit proper documentation to reflect my name changes.
If you have answered NO
to any of the four above statements...
DS-5504, U.S. Passport Re-Application Form is a new form to be used within one year of passport issuance for name-changes and any other descriptive information changes, as well as for the extension of validity for limited passports and correction of any printing errors. The DS-5504 must be submitted with the passport to be replaced, the documentation required for the change, and new photos. There is no charge for routine processing of these changes. After one year of the passport issuance date, Form DS-82, Application for Passport by Mail, or Form DS-11, U.S. Passport Application, as appropriate, must be submitted and all applicable fees paid.
This form should be submitted along with the DS-11: Application for Passport and brought with you when you go in person to apply for a new passport if your previous passport has been lost or stolen.
