Description of Services and Eligibility Requirements
Assisted Living - promotes a resident's self-direction and participation in decisions that emphasize individuality, privacy, dignity and choice in homelike surroundings.
Care Management - consumer-focused collaborative approach that asseses individual needs, then plans, implements, coordinates, monitors and evaluates options and services using formal and informal resources to meet those needs.
Caregiver Assistance Program (CAP) - Supplements the assistance an individual receives from his/her natural support network of family, friends, and neighbors as well as from community agencies and volunteer groups.
Community Care Program for Elderly and Disabled (CCPED) - Designed to help eligible individuals to remain in the community or to return to the community rather than be cared for in a long-tem care facility or a hospital setting.
Information and Assistance - in-office staff assists callers with needed information.
Jersey Assistance for Community Caregiving (JACC) - provides in-home services to seniors at risk of placement in a nursing facility.
Nutrition - Senior Cafés Congregate nutrition senior lunch program for all 60 years and over and Meals-on-Wheels home delivered meals for elderly individuals or couples who are unable to provide their own meals.

Older Worker Program - assists all mature people over the age of 55 in job search and resume writing with the ultimate goal of employment.
Outreach - Reaching out into communities to help access services (see events).
Pharmaceutical and Utility Assistance - PAAD, Lifeline, HAAD and Senior Gold Programs.
Statewide Respite Program - Designed to provide assistance to families by giving caregivers a break from the daily responsibility of caring for an elderly or disabled relative or friend.
Transportation - Union County's Paratransit system provides medical and recreation transportation to seniors and disabled persons. One week advance notification is required. |
