JACC -
Jersey Assistance for Community Caregiving

A program that provides in-home services to seniors at risk of placement in a nursing facility. JACC includes an array of services designed to supplement assistance given by the individual’s caregiver network of family, friends, and neighbors. JACC is a cost share program. Provides a variety of in-home services and supports within the cost cap available to each individual determined on a sliding scale basis

Eligibility Requirements

Must be:

1. At least 60 years old   
2. Not financially eligible for Medicaid or Medicaid Waiver service.
3. Has a cost-sharing obligation of less than 100% of service costs.
4. Is a U.S. citizen or has proof of qualified alien status.

Income Guidelines

$3,165/month Single
$4,260/month Married

Allowable Assets

Single individuals $40,000
Married (couples) $60,000
