Community Care for the Elderly and Disabled program is designed to help eligible individuals to remain in the community or to return to the community rather than be cared for in a long-term care facility or a hospital setting by use of specific home and community-based services and informal supports. Provides specific services with a per-month cap. Must need high level of care. Services Provided: Case Management, Home Health Aide/Homemaker Services, Medical Day Care/Social Day Care, Non-emergency medical transportation, Respite care and prescribed drug.
Eligibility Requirements
Must be:
Age 65 or over and be eligible for Medicare or have other health insurance that includes hospital and physician coverage.
Individual under age 65 must be determined disabled by the Social Security Administration.
Has received a pre-admission screen by the Department of Health and Senior Services for clinical eligibility
