Care Management

Care Management is a consumer-focused collaborative approach that assesses individual needs, then plans, implements, coordinates, monitors and evaluates options and services using formal and informal resources to meet those needs. Promotes choice and dignity.

Eligibility Requirements

Must be:

1. Currently residing in New Jersey
2. At least age 60
3. In need of assistance to access and/or coordinate services
4. Not receiving comprehensive care management services
5. Individuals needing multiple services
6. Persons without available informal social supports such as family or friends
7. People most vulnerable or “at risk” of institutionalization

Priority is given to persons for whom preventive services may avoid or delay the need for institutionalization.
